Clause 11.3.2 of IEEE 1547.2-2023 explains that whether an open phase is detected depends on where the measurement is taken and on the interconnection transformer connection, and clause 11.3.2.3 describes commissioning practice: the test is made at the reference point of applicability, or at an agreed point from which zero-sequence continuity to the PCC is preserved, with every DER device operating, voltage, current and time captured on all three phases at once, and the phase restored while the DER stays connected.
